Caregiver University: Finding the Right Care for You Pt. 2 – Personal Care Plan and Control Journals

An installment of IU Health’s Alzheimer’s education series. This session will cover how to create written documents for relaying care preferences and important information to those on your “care team.” During the session we will create an organizational system for important information that you can begin using right away.
